Centered Living Psychological Services does not work with any healthcare insurance companies and is therefore an out-of-network provider that could cost you more than working with an in-network provider. It is your right to receive a “Good Faith Estimate” that explains the costs of services that are reasonably expected for your health care needs. The estimate will be based on the information known and provided at that time. The Good Faith Estimate will not include any unknown or "surprise" costs that may arise during your treatment. You could be charged more if there are complications or special circumstances occur. In this case, it is your right under federal law to dispute (appeal) the bill.
You can request that your health care provider give you a Good Faith Estimate in writing at least 1 business day before your medical service. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service.
You can dispute a bill that is at least $400 more than your Good Faith Estimate.
Save a copy or picture of your Good Faith Estimate for your records.
